I have tried an experiment at my HMO. I began at some point early in my interaction with the doctor using the title "Doctor" just once in reference to him or her. I notice that my provider's eyes lit up. Then I omitted doing it and went back and forth. When I did it, I had better conversations with the doctor. They worked hard for that title, they like it when you use it.
